The following video recording permissions form templates have been reviewed by Teacher Standards and Practices (TSPC) and transferred to Pacific University letterhead. Please feel free to download and modify as needed:

Video permissions forms are also available as Google Form templates. To get to these templates, be sure you are logged in to Google with your Pacific account, and then navigate to https://docs.google.com/forms/ and click on ‘template gallery’. Note: Once you have an editable copy of the Google Form, be sure to go to ‘settings’ and ‘responses’, and un-select the option that restricts the form to Pacific University users.
